2023成都市工伤补贴申领满足程序有哪些 |

一、成都工伤补贴申领流程
工伤职工劳动能力鉴定结论下达后，用人单位到医保经办机构办理;市人力资源和社会保障局做出老工伤人员纳入统筹管理结论后，用人单位到医保经办机构办理。
二、办理材料
1、工伤人员本人有效身份证正、反面复印件;
2、用人单位应提供注明双方解除或者终止劳动合同原因的书面情况说明(加盖用人单位公章);
3、若属于工伤人员本人提出与用人单位解除劳动合同的，应提供有关证明材料(如辞职报告等)及复印件;
4、若属于45周岁以上女性工伤人员的，用人单位应提供该工伤人员是否从事管理或技术岗位的书面情况说明(加盖用人单位公章);
5、工伤人员本人在本市指定金融机构范围内开立的实名制结算账户卡(折)复印件(可选择邮政储蓄银行、工商银行、农业银行、建设银行、中国银行、招商银行、民生银行、交通银行、其中之一，其中邮政储蓄银行、工商银行、农业银行、建设银行、交通银行已支持异地银行卡折发放);
6、用人单位若提出已垫付部分工伤保险待遇的，需携带由工伤人员签收的已垫付的相关凭证;
7、委托他人办理的，需携带委托书和被委托人的有效身份证原件及正、反面复印件(用人单位申领的无需携带)。
用人单位办理的，材料复印件由用人单位加盖公章;个人办理的，材料复印件由申请人签名。
注：为确保及时办理社保业务，参保人员应提供具有结算功能的个人实名制银行账户，具体情况可咨询您账户所属的银行。
