释义	纳税人丢失发票领购簿或购票密码，应填写《发票领购簿或密码挂失登记表》到主管税务机关发票窗口办理挂失，自受理之日起的5个工作日后，纳税人可以凭回执到主管税务办理相关手续，发票窗口人员应将纳税人的《发票领购簿或密码挂失登记表》及回执作为备案资料归档。因此，按照上述规定，纳税人应填写《发票领购簿或密码挂失登记表》到主管税务机关发票窗口办理发票领购簿挂失，自受理之日起5个工作日后凭回执至主管税务办理重新核发相关手续。法律依据《中华人民共和国发票管理办法》第十五条
