不服交通事故责任认定书怎么申请复核？ |

对交通事故责任认定书的复核应需要采取书面形式递交申请。
根据规定，当事人对道路交通事故认定有异议的，可以自道路交通事故认定书送达之日起3日内，向上一级公安机关交管部门提出书面复核申请。复核申请应当载明复核请求及其理由和主要证据。可见，启动复核程序，有五个限制性条件：
1、时间条件，自交通事故责任认定书送达之日起3日，超期不会处理；
2、形式要求，递交书面复核申请，不能采用口头或其他形式；
3、递交部门，做出事故认定交管部门的上一级公安交管部门；
4、内容要求，复核申请应当载明复核请求及其理由和主要证据；
5、次数限制，只能复核一次，不能复核一次后认为不公正而再次申请复核。
【法律依据】:
《道路交通事故处理程序规定》 第七十一条 当事人对道路交通事故认定或者出具道路交通事故证明有异议的，可以自道路交通事故认定书或者道路交通事故证明送达之日起三日内提出书面复核申请。当事人逾期提交复核申请的，不予受理，并书面通知申请人。
复核申请应当载明复核请求及其理由和主要证据。同一事故的复核以一次为限。
