释义	入院记录是由接诊医生撰写，不能修改，否则就属于篡改病历记录，如果认为医生撰写记录口述出现错误，可以要求与医生再次陈述，医生会在病史记录上予以补强和修正，但对于既往已经记录的错误，撰写后是不能修改的。法律依据：《医疗纠纷预防和处理条例》第十五条规定，医疗机构及其医务人员应当按照国务院卫生部门规定，填写并妥善保管病历资料。因紧急抢救未能及时填写病历的，医务人员应在抢救结束后6小时内补记，并加以注明。任何单位和个人不得篡改、伪造、隐匿、毁灭或者抢夺病历资料。
