深圳市餐饮健康证办理流程 |
| 释义 |
法律分析：按照卫生监督机构确定的人员名单到健康体检单位进行预防性健康检查，统一使用卫生部制定的《中华人民共和国预防性健康检查用表》。
办理健康证前先进行健康查体。
1、带身份证和个人免冠一寸彩色照片2张，到收费处缴纳体检费，领取体检表，粘贴照片(另一张领健康证用)、填写好姓名、性别等基本情况。
2、检验室采血、大便肛拭化验。
3、内、外科常规体检。
4、x光胸透。
5、完成全部体检项目。
查体结果符合发放健康证条件的，经卫生监督机构组织的卫生法律法规知识培训合格后，发放健康证。查体结果不符合发放健康证条件的，不予办理健康证。
法律依据：《中华人民共和国食品安全法》 第四十五条 食品生产经营者应当建立并执行从业人员健康管理制度。患有国务院卫生行政部门规定的有碍食品安全疾病的人员，不得从事接触直接入口食品的工作。 从事接触直接入口食品工作的食品生产经营人员应当每年进行健康检查，取得健康证明后方可上岗工作。
