法律分析：1、填写《出生医学证明自填单》所在医院出具，有部分内容是医生填好的，个人填写部分(填错可以划掉重写)，填写完毕后，可以去换正式出生证明。2、当收到《出生医学证明》后要认真核对。如发现有填写错误时，应及时向医院申请换发。《出生证》全部机打，并且内容“原则上不能更改”(这句话是自填单里的原话，好像听起来还是有可以修改的余地，但是还是不建议冒险)。3、《出生医学证明》是婴儿的有效法律凭证，要妥善保管。4、为了保证出生医学证明有效性，保存时候应该待印章晾干后妥善保存，不可折叠，撕开，或过塑。
法律依据：《中华人民共和国户口登记条例》
第二条 中华人民共和国公民，都应当依照本条例的规定履行户口登记。现役军人的户口登记，由军事机关按照管理现役军人的有关规定办理。居留在中华人民共和国境内的外国人和无国籍的人的户口登记，除法令另有规定外，适用本条例。
第七条 婴儿出生后一个月以内，由户主、亲属、抚养人或者邻居向婴儿常住地户口登记机关申报出生登记。弃婴，由收养人或者育婴机关向户口登记机关申报出生登记。
