上户口要求什么材料和流程 |

法律分析：新生儿落户为自愿塬则，可以随父亲，也可以随母亲，有夫妻商议决定，法律并无特别的要求。上户所需材料：新生儿父母的书面申请、新生婴儿的出生医学证明、结婚证，未婚生育的需要提供监护人的身份证、其父或其母的户口簿，向户籍地公安派出所申报出生登记即可。
1、医院出具婴儿《出生医学证明》，复印一份。
2、父母携带好身份证，户口簿，身份证(复印件)到当地派出所。
3、入户地街道计划生育工作机构出具的计划生育证明(塬件1份)。
4、生育一孩的，需要提供母亲的计划生育服务证;如果是生育二孩的，需要提工单位开具的二胎审批表。
5.超过一个月申报，提交婴儿未随户口在外市县父或母一方入户的证明(塬件1份)。

第十七条 户口登记的内容需要变更或者更正的时候，由户主或者本人向户口登记机关申报；户口登记机关审查属实后予以变更或者更正。
户口登记机关认为必要的时候，可以向申请人索取有关变更或者更正的证明。
第十八条 公民变更姓名，依照下列规定办理：
一、未满十八周岁的人需要变更姓名的时候，由本人或者父母、收养人向户口登记机关申请变更登记；
二、十八周岁以上的人需要变更姓名的时候，由本人向户口登记机关申请变更登记。
