释义	法律分析：在婴儿出生30日内，由婴儿母亲向婴儿出生地妇幼保健站首次申领《出生医学证明》。申请领取时，除提供本规定第十六条第二款规定的证件外，还应当提交婴儿的接生记录证明书、亲子关系声明、婴儿父母双方的居民户口簿、婴儿出生地社区、村(居)委会或单位证明等证明材料。签证机构经调查核实后仍难以查清婴儿出生情况的，应当要求申领人提供当事人的亲子鉴定书。出生证明应先凭借夫妇身份证，在婴儿分娩医院领取《出生医学记录》，然后在产妇户口所在地的妇幼保健院(所)领取《出生医学证明》即可。
