孩子出生的时候，医院会开一份出生证明，出生证明上要有孩子的名字，去为孩子上户口的时候需要出生证明的。但现实生活中，经常出现出生证明与户口本名字不一样的情况，那么出生证明上的名字和户口本名字不一样怎么办？
出生证明上的名字和户口本名字不一样怎么办
出生医学证明一经签发，证件上的各项信息原则上不应变更。如遇出生医学证明不符合户籍登记要求的，须凭户籍管理部门出具的《出生医学证明信息变更建议书》到原签发单位办理变更。如符合户籍登记要求的，不予变更。
如果当事人想改户籍上的名字可以到户籍管理部门进行更改，公民申请变更姓名，需持下列证明材料：
1、申请人的书面申请(16岁以下的，由其监护人提出申请)；
2、父母离异变更16岁以下人员姓氏的，需提供离婚证明手续，并经父、母双方写出协议书签字同意。
3、户口簿、居民身份证；
4、属机关、团体、学校、企事业单位工作人员的，须提交单位、学校人事部门准予变更证明，属在校学生的，应提交所在学校准予变更证明，无单位的需由户口所在地居(村)委会出具证明，然后由户籍所在地责任区民警在证明上签署意见，派出所主管所长同意后打印变更更正表(16岁以下人员变更名字，不需打印变更更正表，由主管所长审批后直接在派出所户籍室办理)报分局户办大厅审核批准。
