1、经营单位：
a、企业注册地的租赁合同、产权证和租赁经营场所经公安机关消防机构验收合格的“建筑消防验收合格意见书”或者“当地消防监督检查意见通知书”(派出所专用)的复印件;
b、单位主要负责人、安全管理人员及其它从业人员的培训证书复印件;
c、企业的工商营业执照、组织机构代码证复印件;
d、老的安全评价报告原件;
e、贵公司近三年内的销售台账。
2、储存单位(注：如果仓储单位由**国际提供，贵公司将不用再提供仓储单位的以下材料)：
a、经营公司与储存单位签订的仓储租赁协议复印件
b、仓储单位取得的当地安全生产监督管理局核发的：“当地危险化学品储存备案证明”;
c、储存单位的危险化学品存储备案证明复印件;
d、储存单位的公司营业执照、组织机构代码证、税务登记证的复印件。
3、运输单位(注：如果运输单位由**国际提供，贵公司将不用再提供运输单位的以下材料)：
a、经营公司与委托运输单位签订的运输协议复印件;
b、运输单位的危险化学品道路运输许可证复印件;
c、运输单位的公司营业执照、组织机构代码证、税务登记证的复印件;
d、运输单位从业人员上岗证复印件。
