亲属关系证明公证材料准备：一、申请人材料：身份证原件；户口簿原件，集体户籍的申请人提供《常住人口登记卡》本人页原件及经过户籍所在单位盖章的首页复印件；二、关系人材料：身份证、户口簿。关系人移居国外并加入外国国籍，提供关系人国外身份证及护照的影印件；关系人常居国外尚未加入外国国籍，但国内身份证、户口簿已注销，需提供关系人的中国户籍注销证明、护照影印件和国外拘留证明；关系人已死亡的，需提供关系人的死亡证明。三、申请人的户籍所在地的公安部门或人事档案部门所出具的申请人与关系人之间的亲属关系证明；特殊：发往澳大利亚使用的父母亲申请与子女的亲属关系公证，需提供父母亲有且仅有几个子女的亲属关系证明。四、证明父母与子女的关系，可以根据自身情况从以下两种证明材料中任选其一代替证明信：如果您以前办理过出生公证或亲属关系公证，可以提供原出生公证书或亲属关系公证书；《出生医学证明》或《独生子女证》，同时提供父母结婚证。五、如非本人办理，还应提供授权委托书、代理人的身份证件；六、如果您所提供的证据材料不足以证明所申办公证事项时，请根据公证员的要求补充证明材料。

《公证程序规则》 第十八条 自然人、法人或者其他组织申请办理公证，应当提交下列材料：（一）自然人的身份证明，法人的资格证明及其法定代表人的身份证明，其他组织的资格证明及其负责人的身份证明；（二）委托他人代为申请的，代理人须提交当事人的授权委托书，法定代理人或者其他代理人须提交有代理权的证明；（三）申请公证的文书；（四）申请公证的事项的证明材料，涉及财产关系的须提交有关财产权利证明；（五）与申请公证的事项有关的其他材料。
