派出所怎么才给开出生证明信 |

（一）如果是在合法医疗机构出生，办理《出生医学证明》者需提供下列材料：
1、原接生机构未发放《出生医学证明》的证明;
2、《婴儿出生记录》的复印件;
3、村证明或居委会证明;
4、夫妻双方的户口本和身份证;
5、办证人的身份证。
（二）如果因遗失、被盗等原因，办理《出生医学证明》者需提供下列材料：
1、由原发证机构提供《出生医学证明》存根的复印件，并加盖公章;
2、由所在地村委会(居委会)或所在单位出具的遗失声明;
3、当地派出所出具的没有进行户口登记的证明。
4、已办理户籍手续后遗失《出生医学证明》者，提供上述1、2款材料，只补发《出生医学证明》正页。
（三）如果在非法机构出生的婴儿，办理《出生医学证明》者需提供下列材料：
1、到就近乡镇卫生院领取《亲子关系声明》，用钢笔或中性水笔认真书写;
2、提供接生机构《医疗机构执业许可证》正本复印件、副本原件、副本复印件;
3、接生人员《乡村医生执业证书》或《执业医师资格证》原件、复印件1份及近期照片一张;
4、非医疗机构人员接生的，需提供接生人员身份证原件、复印件和近期照片一张。
出生证明也就是出生医学证明，为孩子办理出生证明是一件很重要的事情，之后给孩子上户口等，其实都是会需要用到出生证明的。如今孩子一般都是在医院出生的，要是在除开医院的地方出生，那么办理出生证明的时候要求提供的材料就不太一样。假如之后不慎将出生证明遗失，根据规定也是可以要求补发。

《民法典》
第十五条 自然人的出生时间和死亡时间，以出生证明、死亡证明记载的时间为准;没有出生证明、死亡证明的，以户籍登记或者其他有效身份登记记载的时间为准。有其他证据足以推翻以上记载时间的，以该证据证明的时间为准。
